Hvordan kan vi best forstå og behandling personlighetsforstyrrelser?

​Vår kompetansetjeneste samarbeider med en forskergruppe som er tilknyttet avdeling for personlighetspsykiatri ved Oslo Universitetssykehus. Forskergruppen har valgt å konsentrere seg om de hyppigst forekommende personlighetsforstyrrelsene i klinisk praksis: Emosjonelt ustabil personlighetsforstyrrelse (PF) og unnvikende PF. En har også valgt ut de områder som ser ut til å ha størst betydning for personlighetsfungering: sosial kognisjon, følelsesregulering og interpersonlig relatering.

På disse nevnte områdene er det investert mye i forskningsmetodikk: Kompetanse i vurdering av tilknytningsmønster og refleksiv funksjon (sosial kognisjon), kompetanse i vurdering av følelsesbevissthet og følelsesregulering, fysiologiske mål på følelsesaktivering, kompetanse i psykometri for å kunne vurdere indirekte mål på sosial kognisjon, tilknytning, følelsesregulering og interpersonlige relasjoner, samt kompetanse i vurdering av terapeuters måte å håndtere følelser, mentalisering og interpersonlige relasjoner i så vel individualterapi som gruppeterapi.

Forskergruppen har med dette et ambisiøst langsiktig program som er integrerende med hensyn til forskjellige fagdisipliner og hvor grunnforskning og behandlingsforskning henger tett sammen. Forskergruppen har et nært samarbeid med en spesialisert psykiatrisk poliklinikk og 16 andre spesialiserte kliniske enheter i Sør-Norge via Dagbehandlingsnettverket.

    • Det er et tett og gjensidig samarbeid mellom klinikere ved Personlighetspoliklinikken, forskningsgruppen og leder for DagbehandlingsNettverket. NAPP er involvert i en rekke forskjellige forskningsprosjekter. I forskningsgruppen ved Seksjon for personlighetspsykiatri som ledes av Elfrida Kvarstein, seksjonsoverlege ved Seksjon for personlighetspsykiatri/1.amanuensis UiO,foretas kontinuerlige drøftelser av fremtidige forskningsstrategier og prosjekter som er relevante for personlighetspsykiatrien. Det store multisenterprosjektet om den alternative modellen i DSM-5 dreier seg om en bedre og mer valid diagnostikk av personlighetsforstyrrelser enn dagens diagnosesystem ICD-10 og DSM-IV gir. I dette prosjektet er HSØ, HMN involvert og vi har planer om å involvere HN. I dette prosjektet har også professor Adrew Skodol (USA), som var leder for denne alternative modellen og professor Donna Bender fra USA som har vært sentral i å utforme Levels of Personality Functioning Scale, som er en sentral og nyskapende del av modellen, vært i Norge og deltatt på workshops knyttet til prosjektet. Forskere fra Danmark og Sverige er også involvert i felles workshops om denne modellen. NAPP vurderer dette prosjektet som sentralt for kompetansetjenestens ansvarsområde. Prosjekter knyttet til utforskning av mentaliseringsbasert behandling foregår også i flere helseregioner og ses på sentralt med henblikk på kompetanseøkning av behandling for personlighetsforstyrrelser (PF) og PF i kombinasjon med rus. Det er nær kontakt med Anna Freud Centre i London i videre utvikling av mentaliseringsbasert behandling, og man søker å publisere sammen med forskere derfra i fremtiden. Unnvikendeprosjektet på Seksjon for personlighetspsykiatri er viktig med henblikk på å utvikle spesifikk behandling for denne pasientgruppen som har store funksjonsproblemer i tillegg til mye subjektiv lidelse. Det er svært lite internasjonal forskning på unnvikende PF.
